UDID — disability certificate and identity card services
The UDID system supports applications for disability certificates and Unique Disability ID cards, with assessment by the competent medical authority. The department describes online registration, application tracking, renewal and replacement services. Applicants can also find the relevant medical or welfare authority. An online submission does not itself establish a disability classification or guarantee a certificate.
